A. Submitted to the Municipality
1. Application by the interested lessor (natural or legal person) or third party with authorization.
2. Photocopy of the lessor’s police ID card.
3. Photocopy of the lessee’s police ID card. If the applicant is a foreigner – a citizen of a non-EU country – a photocopy of a valid residence permit allowing them to engage in independent economic activity. If the person concerned is a member of the Greek diaspora, a photocopy of their special diaspora identity card.
4. Lease agreements in two (2) copies with the signatures of the lessor and lessee certified by a public authority.
5. Affidavit of Law 1599/1986 of the previous tenant (if any), stating that they voluntarily vacated the kiosk before the end of the lease. Informative note from the General Accounting Office of the State regarding the last three months’ war pension (in the case of a disabled beneficiary).
7. Copy of the kiosk’s electricity bill. Municipalities and communities are responsible for issuing licenses for the establishment and operation of health-related establishments, internet service providers, and canteens within public buildings whose activities are not related to the needs of the building.
ATTENTION:
v In order for the licensing process to begin immediately, the following supporting documents must be submitted together.
v The responsible statements (of N1599/86) and authorizations must be certified for the authenticity of the signature by a Public Authority.
B. In order for the Health Service to issue a food sales license for kiosks, the following supporting documents are required:
• Application (describing in detail the types of food the applicant wishes to sell)
• Photocopy of the kiosk license
• Photocopy of the lease permit (Municipal Decision)
• Photocopy of the local authority permit for the location of the kiosk and occupation of the surrounding area
• Fee of €88 payable to the Region at the Bank.
• Floor plan showing the outline of the kiosk (hut) and any outdoor areas (to scale). The plan shall show the location and layout of food counters and refrigerators. In the case of complex outdoor structures, a sectional plan shall also be submitted to show the layout of any structures.
• Simple flow chart (receipt – inspection – placement – exposure).
In the event that the municipalities decide and grant permission to use outdoor areas around the main kiosk for the display of food and other items with additional structures beyond the main kiosk awning (e.g., extensions with additional tents or other types of shelters, screens, partitions, etc.) to protect refrigerators or shelves that will be installed outside, beyond the issues that are the main responsibility of each municipality (e.g., requirements for modern design, uniformity, functionality, public safety, aesthetics, etc.), the competent Urban Planning Department of your municipality should clarify whether or not urban planning permits are required. If urban planning permits are required, they must be obtained prior to the Health Service permit for the sale of food.
In any case where there are extensions to the kiosk, they must be submitted to the Health Service together with the other supporting documents and the corresponding documents from the Urban Planning Service (i.e., a copy of the permit or a certificate of non-requirement of permits) as well as floor plans and cross-sections of any additional structures to the main kiosk.
These drawings shall accompany the Health Service permit for the sale of food in kiosks, so that any arbitrariness and excesses can be checked.
The Department of Health Regulations of the Ministry of Health and Social Welfare, to which this document is addressed, is requested to inform us of its opinion on the above.
C. Granting of a license to sell standardized ice cream, soft drinks, and beverages from kiosks for the disabled

Supporting Documents
a. Photocopy of kiosk license (from the prefecture).
b. In the case of a leased kiosk, a photocopy of the prefectural decision granting the right to lease it.
c. Certificate from the relevant municipal authority that the placement of the refrigerators used (2, on the respective sides) complies with the relevant provisions of the law.
d. Photocopy of your Individual Health Booklet (certified).
