Installation permit documents.
Issuance of a building permit in the case of the construction of a new building or modification of the terms of an existing one.
Certificate of land use from the competent urban planning authority—certifies that the specific use is permitted in the specific building block or area.
Approval of environmental conditions – study by a private engineer, in accordance with current legislation and approval by the Prefecture, Region or Ministry of Environment, Physical Planning and Public Works, depending on the category.
Topographical diagram, which also shows the business.
Plans – study of the space, floor plans, sections showing the equipment, installed power, and cost.
Engineer’s statement regarding the structural adequacy of the building and the installation of the equipment.
Questionnaire completed by the business operator.
Remuneration – tax matters.

Operating license documents

Plans – technical description for the Certificate of Suitability from the Health or Rural Development Directorate.
Fire safety certificate from the Fire Department and approved study in accordance with F. 15/oik.1589/104/2006 (unless the facility is exempt).
Responsible statement by an electrician regarding the electrical installation.
Responsible statement by the engineer and owner that the terms of the installation permit are being complied with.

Factories-Laboratories

Installation permit documents

Questionnaire signed by the legal representative of the company and certified for the authenticity of the signature.
Approval of environmental terms with the approved study attached.
Certificate of land use from the competent urban planning authorities.
Technical memorandum in the case of granting a license to establish a professional workshop.
Architectural drawings (topographical, coverage diagram, floor plan, sections) in the case of a building permit.
Risk assessment study, in the case where the activity falls under the category SEVEZO, in accordance with the number number. 5697/2000 (Government Gazette 405B) joint ministerial decision.
Opinion of services or bodies on case 20.g of the questionnaire, if required.
Responsible statement by a civil engineer in the case of existing buildings.

Operating license documents

A solemn declaration by the operator or its legal representative and, where applicable, by engineers of the relevant specialization, that the installation was carried out in accordance with the installation permit granted,
and that the planned works were carried out in accordance with the approved studies.
A solemn declaration by the representative of the entity, stating in detail how many technicians of which specialties will be employed in accordance with the law.
Responsible statement by the competent person(s), in accordance with the provisions in force, technician(s) who has undertaken the legal supervision of the operation and maintenance of the mechanical equipment of the installed activity.

Special supporting documents (if required, on a case-by-case basis)

Fire protection certificate.
Permit for the disposal of liquid industrial waste.
Building permit, if required by the installation permit, certified upon completion of the work.
Hydraulic test and boiler acceptance certificates, in accordance with the applicable legislation.
Water use permit, in the case of water use from drilling for industrial purposes.
Certificate of construction of a traffic connection, in cases where the facility’s grounds face a national highway.
Certificate of suitability from the competent Health Authority for the application of Article 52 of Health Regulation 8577/1983, Presidential Decree 369/1992, and Health Regulation
A5a/ 5333/1987 (Government Gazette 721 B), as applicable.
For multi-story buildings, co-ownership regulations in accordance with the provisions of Article 4 of Law 3741/1929 (Government Gazette 4 A), “On ownership by floor.”
